クロスサイトリクエストフォージェリ対策 
一般にCSRFはセッションの利用に伴う脆弱性なのだから、予測不能な文字列を生成してhiddenとセッション内(PHPセッションの場合$_SESSION内)に入れておき、完了画面でこの2つを照会する方法が簡便で堅牢だと考えられる。(要はセッションを使ったワンタイムトークン方式)。
この対策だけでも充分だが、hiddenを横取りされた場合に発生する脆弱性に備え、Cookieでも比較を行う。

堅牢性
- セッション固定攻撃的手法
- 攻撃者はサーバのセッション内に偽のIDをセットすることができないので無効。
- hiddenの横取り
- hiddenは偽装できるが、攻撃対象者UAのCookie内にその値をセットしなければ攻撃が成立しない。
この対策が無効な場合
攻撃者が善意のユーザのUAを操作し、実際の確認画面を経て完了画面に遷移させるような場合はこの対策は無効となる(このような場合、現在行われているほとんど全てのCSRF対策は無効となる)。このようなことが可能であるならばそれはUAの深刻な脆弱性となる(CSRFどころの騒ぎではない)ので実際にこのようなことが起こるとは考えにくいが、あらゆる可能性を考慮するならば完了画面で再度認証を行うべきである。

こんにちは。
あまり自身がないんですが、Cookieに吐くIDとhiddenに入れるIDを違うものにして、セッションデータに2つ（正確には片方がセッションID）を保存したほうが、より強いと思うのですが、どうでしょう。
つまりクライアントには２つの異なるID、「ペア」を持たせる、ということです。
IDが同じだと、たとえhiddenとCookieに保存させても、どちらか片方だけ入手できれば良い訳で、たとえば何らかの方法で攻撃者がCookieを盗むことができたら、送信前（IDの有効期間）にターゲットを攻撃用フォームにアクセスさせれば攻撃が成功してしまいます。
まず一回、Cookieを吐くページにターゲットを暗黙的にアクセスさせるとか。（この時点でCookieとセッションに保存される。）
まったく関連性のない2つのID「Ａ と Ａ’」を生成して、その組み合わせをセッションに保存、すなわち「Ａ と Ａ’ がペアである」ことを知っているのはサーバーだけ、ということになるので、処理ページで、送られてきたhiddenのIDをセッションIDとして読み出して、そこに書いてあるもうひとつのIDがCookieに入っていればOKと判断、という考え方です。
まぁこれでも、CookieMonster的な何らかの方法でCookieにセットさせることができてしまうなら、あらかじめ「正しいペア」を入手されてしまえばアウトですが…。
思いつきに過ぎないんで、間違ってたら済みません。
